It is with mixed emotions that I announce my retirement. I will be closing my office on

If you need pain management, you may reach out to the physicians identified here. You may also contact your insurance company to identify another provider or ask you primary care provider for a recommendation. Your medical records are confidential, and a copy can be transferred to another provider, or released to you, or another person that you designate only with your permission. Please let us know as soon as possible where you would like your records to go. I have included a medical release form that will allow me to share your records with the next provider. I can not release those records without that release form.
